Student Release Agreement 

I understand that Eden Energy Medicine is an innovative approach to health care, which is only beginning to be investigated scientifically. I further understand that the training in Eden Energy Medicine I may be receiving from Calming Stress and Anxiety with Eden Energy Medicine/Innersource does not substitute for diagnosis and/or treatment from a qualified health practitioner for illnesses, injuries, or other medical conditions.

As a part of this class, I may be touched briefly to demonstrate energy testing, such as light contact on the arm or hand to check the body’s energy response. I consent to this brief contact, and I can decline or ask to stop at any time.

I also understand that any activity in which a person might participate involves some risk. I release and hold Calming Stress and Anxiety with Eden Energy Medicine / Innersource, along with its agents, officers, employees, directors, successors, and affiliated corporations harmless from claims arising out of my participation.

I understand that I will receive a short survey by email about this workshop. This email will not be used for marketing.
